Preventive Case Planning Services

Our Preventive Case Planning program serves families with children and youth ages 0-18, incorporating the Solution-Based Casework (SBC) model as well as Motivational Interviewing (MI). SBC is an evidence-based and solution-focused intensive family preservation framework that is complemented by the MI tool for counseling individuals and families in identifying motivations that can lead to impactful behavior change.

Goals of PCPS

The primary goal of this program is to maintain at-risk children’s safety in their homes and communities by engaging families in community-based interventions in the least-restrictive setting. We aim to keep families together by preventing out-of-home placement. We:

→   Assist families in building skills, attaining goals, and achieving lasting permanency.

→   Craft a service plan with the County Case Manager and the families that connects families to community-based services and facilitates effective family-provider partnerships.

→   Ensure that families basic needs are met at all times, including shelter, food, transportation, and clothing. 

→   Utilize relentless family engagement and stand by families as they navigate any number of challenges.

→   Find the community-based services with the highest likelihood of assisting families in achieving their permanency goals.

→   Meet with families in their homes twice a month.

→   Provide 24/7 support.

Who is eligible for this service?

→   Families receiving PCPS services are court-ordered by CPS.
