Health Home Care Management (HHCM)

A FREE program for Medicaid-eligible children and adolescents aged 0-21 that assists families, prevents emergency room visits and hospitalizations, and improves school attendance.

Health Home Care Management helps families manage the health and well-being of children and youth aged 0-21 who are enrolled in Medicaid. The program promotes the health of children and families while reducing emergency interventions, truancy, and hospital care. This program is designed to ensure that children receive the care they need to be healthy and happy, and families can feel confident in managing their child’s health.

Better access to your Child’s Medicaid Health Services

Health Home Care Management gives families:

✓   A designated Care Manager
✓   Coordination of service needs for complex  mental, behavioral, or physical health situations
✓   Creation of a comprehensive service plan
✓   Transportation assistance
✓   24/7 Availability

Who is eligible for this service?

→   Children ages 0-21 enrolled in Medicaid
→   Children who have two or more Chronic Conditions

OR any one of the following:

→   HIV/AIDS
→   Serious Emotional Disturbance (SED)
→   Trauma
→   Mental Health Diagnoses

Who can refer?

→   Parents
→   Teachers and Counselors
→   County Case Workers
→   Anyone who thinks a child or their family may need assistance.

Care Managers will assist in the referral and application process.
