Targeted Case Management

Targeted Case Management (TCM) seeks to create a community and a team who are open to talking about and supporting mental health. TCM offers a family-driven, youth-guided and culturally competent system of care for youth with emotional needs and their families. Our TCM services connect and support youth and their families with community-based services and other valuable resources.

TCM Services

An assigned Specialist and Family Partner will:

→   Explain what “System of Care” is and how it can help a family

→   Work with the family to select a larger team of people to assist with setting goals and determining the best path for reaching them

→   Take direction from the family on what services are right for them

→   Help the family respond to crises and concerns

→   Develop plans that support a family’s areas of need

→   Provide support and training for families to be the leaders and decision makers for their own care

Goals of TCM

→  Create simple and direct connections to services for the youth and family

→  Help families become the leaders and decision-makers for their own care

→  Improve mental health of youth and family

→  Strengthen skills that improve a family’s ability to navigate crises

Families utilizing our Treatment Family Foster Care services that need support accessing resources to support their mental health.
